APPLICATION PROCESS

Steps to apply

Online

  1. Gather your documentation to upload. Click here for DOCUMENTATION GUIDELINES.
    To complete the online application, you will be required to upload documentation. For multiple documentation forms, scan documents into one document. The online application will allow only one upload.
  2. Click below or on APPLY NOW.
  3. Complete the application, upload documentation, and submit.

In Person

  1. Pick up an application and documentation guidelines from the Disability Resource Center.
  2. Return the completed application with appropriate documentation. Please keep a copy of your documentation.

WHAT’S NEXT?

EMAIL NOTIFICATION. You will receive an email verifying the Disability Resource Center has received your application within one business day.

APPLICATION REVIEW. The Disability Resource Center staff will review the application and documentation and determine appropriate action:

  • Requested accommodations approved.
  • Requested accommodations denied.
  • Request for additional documentation.

MEET WITH THE DRC. A DRC staff member will contact you to schedule an appointment to discuss accommodation options, implementation process, and other available support services.
